RECORD OF CAITLIN’S MEDICAL EXAMINATION Caitlin arrived in my surgery complaining of an uncontrollable tingling feeling in her nipples and genitals. On taking a medical history, I was able to establish that this first occurred when she thought about being spanked, and that over time the thoughts and the tingling had become an almost permanent feature of her waking life. I explained to Caitlin that today’s examination would be challenging, painful, intrusive and humiliating. I further sought her consent for certain procedures and demonstrated to her the operation of the medical equipment. I noted that her heart rate increased from a resting 79 to 85 following this explanation. BREAST EXAMINATION I asked Caitlin to remove the clothes on her top half. After putting on latex gloves and cleaning the area with antiseptic wipes, I then applied various stimuli to her breasts and nipples, including the use of my hands, forceps, probe and wartenberg wheel. We were able to establish that Caitlin’s right breast and nipple was considerably more sensitive to painful stimuli than her left. BUTTOCK EXAMINATION I asked Caitlin to remove her remaining clothes and to kneel on the examination couch with her elbows flat, presenting her buttocks for testing. I conducted two tests for stimulus response: the first involved the use of a wooden hairbrush, and the second a short leather strap. Caitlin reported that while the hairbrush was painful, the leather strap stung more and left a more lasting sensation. By this stage I was able to see some redness showing on Caitlin’s buttocks. I further tested her responses by touching and stimulating the external part of Caitlin’s anus. Finally, I conducted a test for sensory conflict, asking Caitlin to apply the magic wand vibrator to her genitals while I deployed a single strand silicon whip to her buttocks. This is an exceptionally painful implement and at first it appeared that the pain would win out over pleasure. However, Caitlin turned up the speed on the wand and was able to climax satisfactorily. By this stage, the amount of redness to Caitlin’s buttocks was very pronounced. GENITAL EXAMINATION (EXTERNAL) I asked Caitlin to lie on her back with her legs wide open. I then examined the external part of her genitals, using my gloved fingers, the forceps, probe and wartenberg wheel. Caitlin experienced some pain when the probe was applied to her urethral opening. GENITAL EXAMINATION (INTERNAL) I applied generous quantities of lubricant to Caitlin’s vagina using a specialised syringe. Following a thorough manual examination, I inserted a clear plastic speculum into Caitlin’s vagina and opened it to its full extent. With the aid of a built-in light, some photographs have been taken to enable more leisurely study. I then invited Caitlin to insert and open a metal speculum before applying vibration to the external part of the equipment. Caitlin found that with the metal speculum fully open, she was able to experience the same pleasure from the magic wand, but that it was more difficult to climax. This proved to be a temporary problem, and Caitlin reported that she had found the experience most satisfactory. CONCLUSION After checking the extent to which the use of a speculum had enabled Caitlin to “gape” open her vagina, I concluded that the only course of action now left to me was to fuck her. Which I did. DIAGNOSIS After examining Caitlin thoroughly and conducting the tests set out above, I have come to the conclusion that the tingling feeling that she feels is entirely normal given the thoughts that she has.
